On Task Self-Monitoring Checklist
Written by Adi Ackerman, Head Teacher
"On Task Self-Monitoring Checklist" is written for Kindergarten to 5th Grade (Ages 5-11). Children a grade either side can use it too: younger ones with an adult reading it aloud, older ones as a quick refresher.

On task self-monitoring checklist for K to 5th grade: 6 on-task behaviors with pictures, checked at 6 timed intervals during work. 1 printable page. Part of our behavior trackers collection.
How do I use this resource?
Set a quiet timer for short intervals during independent work. Each time it goes off, the student checks a box for every behavior they are doing right then. Count the checks out of 36 together and set a target for next time. It builds self-awareness for students with attention or focus goals.
What students will practice
- Turns a behavior or habit goal into something a child can see and track, one sticker, token or check at a time.
- Pairs a clear goal with a reward agreed in advance, so expectations are concrete for the child and the adult.
- Supports positive behavior plans at school and routines and chores at home.

FAQ
How do I use the On Task Self-Monitoring Checklist?
⌄
Set a quiet timer for short intervals during independent work. Each time it goes off, the student checks a box for every behavior they are doing right then. Count the checks out of 36 together and set a target for next time. It builds self-awareness for students with attention or focus goals.
What is the On Task Self-Monitoring Checklist?
⌄
A one-page On Task Self-Monitoring Checklist. Six on-task behaviors with small labeled pictures (I am in my seat, I have my materials, I am working on my task, I raise my hand, I use a quiet voice, and I ask for help when I need it) run across 6 checks, with a line to write the time of each check.
Can I use the On Task Self-Monitoring Checklist at home and in the classroom?
⌄
Yes. We made it for Kindergarten to 5th Grade reward and behavior charts, and it works the same at home or at school. Families print it for one child and keep it where they will use it every day. Teachers print a copy per student or one for the whole room, whichever the day needs.
What kinds of reward and behavior charts are included?
⌄
Thirty charts in five groups: sticker charts (10, 20 and 30 spaces, potty training, reading and a themed set), reward charts (I Am Working For, a token board, a 50 point chart, coupons, a class marble jar and a reward menu), behavior trackers, clip charts with name labels and desk strips, and chore and routine charts.
Which grades are these charts for?
⌄
They span Pre-K to 5th grade. Sticker charts, token boards and picture chore charts are made for Pre-K to 2nd, while the 50 point chart, the reward menu, the reflection sheet and the big kids chore checklist suit older students. Each resource page names its own grade span.
How do I make a reward chart work?
⌄
Pick one clear goal, agree on the reward before you start, and mark progress right away when the goal is met. Keep the chart where the child can see it, keep early rewards small and quick, and start a fresh chart when it is full.
How do the clip charts work?
⌄
Each level prints on its own page: cut them out and hang them in order. Every student starts the day on Ready to Learn (or Ready for Launch on the space chart) and moves a clothespin up or down, then starts fresh the next morning. The Positive Clip Up Chart only moves up.
Are these charts useful for students with IEP behavior goals?
⌄
Yes. The token board, the I Am Working For chart, the daily and weekly trackers, the goal chart and the on task self-monitoring checklist all turn a behavior goal into something a student can see and track, and the home and school note keeps families in the loop.
Will the charts print in black and white?
⌄
Yes. They use pastel fills and line art on white, and every level, face and box is labeled with a word or number, so nothing depends on color.
